Post - Coaching Questionnaire

We hope that you have enjoyed your coaching experience with us. Now that you have got to the end of the process we would like to capture some feedback about your programme and how you have found it.

The information that you provide in this form will be presented back to you in the form of an end of coaching report. For confidentiality reasons you will be the only person in your organisation to receive this report, it is your responsibility to then show it to your manager or your organisation. Should you have any confidential information to feed back, that you are not able to include in this form then please send this through to [email protected] or call us on 0345 626 2244.


What happens with this information

Once complete the information from this form will be used with the information from your pre-coaching assessment to create an end of coaching report.  This report will only be sent to you in your organisation and it will be your responsibility to show it to your manager/organisation.  We do this to protect your confidentiality and also make sure you take full ownership of your own development.
